Digitised Patient Care Pathways
Project: SNOMED clinical terminology and clinical health issue coding
A project to standardise and improve the coding of medicines and medicine allergies within the electronic patient record. Also involves a piece of work to tidy up and rationalise the current categorisation of clinical health issues within the EPR. This will then link these to SNOMED codes.
SNOMED is a structured clinical vocabulary for use in the EPR and compliance with SNOMED clinical coding to the current health issues is required by the NHS by 2020.
- Improved clinical coding leading to safer patient care and reduction in hospital standard mortality ratio
- Standardised entry of coded health issues for more accurate reporting
- Compliance with NHS requirements which will improve Trust’s digital maturity rating
